Pre-conceptional counselling means planning your pregnancy in optimum health
state. It is all the more needed for a couple who had a bad obstetric history – it may
include any of the followings:

  • History of recurrent pregnancy losses.
  • History of previous baby with anomalies (abnormalities) or aneuploidies (or
    other genetic diseases).
  • History of genetic disorders in the family (like thalassemia, sickle cell,
    Duchne’s muscular dystrophy, etc).
  • History of medical disorders in mother like hypertension, diabetes, heart
    disease, kidney disease, liver disease, etc.
  • History of surgery in the past like myomectomy, bariatric surgery, etc
  • History of intra-uterine deaths or still born baby.

Periodic and regular checkups are must in any pregnancy. It is ideal to see your doctor as soon as you miss your period and your urine pregnancy test become positive. It is so important because the baby inside is getting formed in the initial three months of your pregnancy and it needs supplements in your diet for her growth especially of brain and spine.

Doctors plan the further visits depending upon your history, examination and some basic tests. Some couples may need frequent visits than usual depending upon mother’s health and baby’s growth. Usually your doctor would like to see you once in every 3-4 weeks time in first and second trimester and more frequently in later pregnancy.

1. Viability scan
This is the first scan done during pregnancy, usually between 7 – 10 weeks of pregnancy.
Aim :
a. Ensure that pregnancy is inside the uterus
b. See the number of embryos present
c. Determine the baby is alive
d. Dating the pregnancy & provide estimated due date (EDD)

2. First trimester screening / Nuchal translucency (NT) scan
This is one of the most important scans during pregnancy and should be universally performed. It is performed between 11 weeks to 13 weeks 6 days.
Aim :
a. Accurately date the pregnancy
b. Look for multiple pregnancy
c. Detect major fetal abnormalities
d. Assess the risk of Down’s syndrome and other chromosomal conditions

3. USG level II / Target scan / Anomaly scan
This scan is done to see in detail all parts of the baby. It is usually done between 19 weeks to 23 weeks. Each part of the fetal body is assessed closely with special attention to brain, spine, heart, lung, stomach, bowel, kidneys, bladder, face and limbs.

4. Fetal well being scan / Growth scan
It is usually performed between 32 weeks to 36 weeks of pregnancy, although it can be performed anytime between 26 weeks up to term. It is done to measure fetal growth , estimate fetal weight, amniotic fluid and fetal movement.

5. Colour Doppler
Colour Doppler examination of blood flow to placenta & fetus is assessed when required. It can be done anytime between 26 weeks up to term.

6. Additional scans :
a. Fetal echocardiography – Detailed scan of fetal heart to rule out abnormalities in heart.
b. 3D/4D scans – To look certain fetal parts in 3D like face, spine, limbs, etc
c. Cervical length screening – This is a trans vaginal scan to measure cervical length. It is recommended in women at high risk of preterm birth including multiple pregnancies, those with history of preterm birth, uterine abnormalities or previous cervical surgeries.

There are few blood and urine tests that are routinely performed in all pregnant women, to see the general health of the mother and also to look for infections which may pass on to the baby. Here is a short list of some basic tests routinely clubbed together as ANC profile

1. Blood Group
2. Complete blood picture / Haemogram
3. HPLC (High performance liquid chromatography / Hb electrophoresis)
4. TSH / Thyroid profile
5. HbA1C / Glycosylated haemoglobin
6. HIV 1 & 2
7. HBsAg
8. VDRL / RPR
9. Anti HCV
10. Urine routine microscopy

There are few genetic conditions where chromosomes are present in triple copies which may hamper the normalcy of fetus, such are called trisomies. Down syndrome is one such trisomy where chromosome number 21 is present in triple copy. These babies have some common ailments like lower IQ, structural heart defects, renal problems, etc.

Each mother is at risk of birthing a Down’s baby, thus it is essential to universally screen all mothers. There are few screening methods which may assess the risk like

1. Nuchal translucency scan (NT/NB sonography) – done from 11 weeks to 13 weeks 6 days
2. Double marker test – blood test, done from 11 weeks to 13 weeks 6 days
3. Triple marker test
4. Quadruple marker test – blood test, usually performed after 16 weeks
5. NIPT (Non invasive prenatal testing) – It is one the most recent and most accurate screening method with 99% sensitivity where cell free fetal DNA is extracted from maternal blood sample, amplified and studied for common trisomies- 21, 13, 18. Also it can identify 22q deletion and sex chromosome abnormalities.

There are other common trisomies like Patau syndrome, Edward syndrome, etc. The risk for these conditions are also assessed by above mentioned tests.

Routine testing for genetic conditions is not done in all mothers. Although some mothers are subjected to specific genetic testing for certain conditions especially if there is a family history or history of previous affected baby. Test will depend upon the history and disease associated. These mothers may need carrier testing of the spouse as well.

Contraception post-delivery is equally important aspect of care as it helps to give required time between two pregnancies which is essential for the mother to regain its reserve and strength for subsequent births. There are numerous choices available in contraception, but it is best to take advise from specialist to choose the best for you.

Our Working Process

Consultation

This includes full check up and advise. Antenatal checkup usually involves check of your weight, pulse rate, blood pressure, temperature, also how big is your baby (is she growing adequately for her dates), etc. It basically guide regarding the change of diet, medicine or need for any investigations.

02.

Investigation

All pregnant women need some basic blood and urine test to know about the general wellness. These are usually clubbed together in a panel known as ANC profile. Apart from these some other tests may be required periodically as directed by her physical examination or underlying medical condition.

03.

Ultrasonography

Ultrasound is an excellent tool which gives insight regarding the health of the baby. Baby can be seen completely inside out, growth can be measured, parts can be examined for normalcy and other things like liquor & blood supply can be monitored. You can read more in our “essential scans section”.

04.

Plan for delivery

Delivery is the most awaited part of whole pregnancy, whole family wait for a healthy little one to come into their lives. Planning delivery is crucial as it involves many factors like previous obstetric history, growth & Doppler of the baby, presentation of baby, liquor around baby, descent of baby’s head during labour, fetal heart rate, etc. The ultimate goal for us is to have a healthy baby in healthy mother’s lap.
