Information may be provided in person, by phone, email, messaging, an available website form, or by a doctor, hospital, laboratory or caregiver where sharing is authorised or legally permitted.
Why we use it
To respond to enquiries, arrange appointments, provide and coordinate clinical care, maintain records, communicate results or follow-up, process administration or payment, protect systems and comply with legal or regulatory duties.
Share only what is needed for the purpose.
Where sensitive health information is collected electronically, Maa Clinic seeks to make the purpose clear and use it for that purpose. You may decline to provide optional information or withdraw consent for future optional processing in writing, although this may limit the clinic’s ability to provide a requested service.
